POSITION SUMMARY

The Social Service Worker (SSW) supports clients of the Adult Day Program and their caregivers through person-centered programming, emotional support, education, and resource navigation. Working as part of an interdisciplinary team, the SSW assists clients in maintaining independence, social engagement, and overall well-being while supporting caregivers through education, advocacy, and connection to community resources. All duties are performed in the best interest of the clients and their caregivers, and in accordance with Woodingford Lodge’s vision, mission and values statement. 

 QUALIFICATIONS:

  • Post-secondary education in social or human services or related discipline
  • Must be a Registered Social Service Worker in good standing with the Ontario College of Social Workers and Social Service Workers.
  • Demonstrated knowledge and ability to complete MoCA, GDS, Zarit Burden Scale, Cohen-Mansfield and any other necessary assessments
  • Experience in counseling, assessment, and crisis intervention with an understanding of client difficulties related to economics, education, victims of family violence, physical or psychiatric impairments, geriatrics, addiction and isolation.
  • Demonstrated ability to identify and collaborate with internal and external partners to deliver programs, services and outcomes.
  • Knowledge of relevant trends and legislation, best practices and regulations including the Fixing Long-Term Care and Occupational Health and Safety Act
  • Ability to demonstrate the required competencies for this position in alignment with Oxford County’s competency framework.
