POSITION SUMMARY:
The Social Service Worker (SSW) supports clients of the Adult Day Program and their caregivers through person-centered programming, emotional support, education, and resource navigation. Working as part of an interdisciplinary team, the SSW assists clients in maintaining independence, social engagement, and overall well-being while supporting caregivers through education, advocacy, and connection to community resources. All duties are performed in the best interest of the clients and their caregivers, and in accordance with Woodingford Lodge’s vision, mission and values statement.
KEY ACCOUNTABILITIES
- Oversee the intake and discharge process for clients and caregivers.
- Build therapeutic relationships with clients and caregivers.
- Complete client assessments and contribute to individualized care plans.
- Monitor client well-being and identify changes in physical, cognitive, emotional, or social functioning.
- Support clients in achieving personal goals related to wellness, social engagement, and quality of life.
- Advocate for clients’ rights, dignity, and choice.
- Facilitate caregiver support groups and education sessions.
- Assist caregivers in navigating community services, long-term care resources, and healthcare systems
- Connect caregivers with appropriate internal and external supports.
- Promote caregiver wellness and respite opportunities.
- Support wellness initiatives, therapeutic recreation activities, and restorative programming.
- Collaborate with Physiotherapy, Recreation, Nursing, and other interdisciplinary team members to deliver holistic care.
- Assist in developing new programs and services that meet client and caregiver needs.
- Develop and maintain partnerships with community agencies and service providers.
- Promote awareness of Family Transitions services within the community.
- Participate in outreach events, caregiver education initiatives, and community presentations.
- Maintain accurate, timely, and confidential documentation.
- Complete assessments, progress notes, care plans, reports, and statistical tracking as required, ensuring all client information is current and accurately reflected in program records.
- Maintain an up-to-date caseload and client identification list, ensuring all client status changes, including medications, diagnoses, admissions, discharges, and service updates, are accurately documented and readily available to support safe program operations.
QUALIFICATIONS FOR SUCCESS
- Post-secondary education in social or human services or related discipline
- Must be a Registered Social Service Worker in good standing with the Ontario College of Social Workers and Social Service Workers.
- Demonstrated knowledge and ability to complete MoCA, GDS, Zarit Burden Scale, Cohen-Mansfield and any other necessary assessments
- Experience in counseling, assessment, and crisis intervention with an understanding of client difficulties related to economics, education, victims of family violence, physical or psychiatric impairments, geriatrics, addiction and isolation.
- Demonstrated ability to identify and collaborate with internal and external partners to deliver programs, services and outcomes.
- Knowledge of relevant trends and legislation, best practices and regulations including the Fixing Long-Term Care and Occupational Health and Safety Act
- Ability to demonstrate the required competencies for this position in alignment with Oxford County’s competency framework.
