POSITION SUMMARY

The Supervisor of Community Programs is responsible for the development, implementation, and evaluation of programs that promote meaningful engagement, social connection, and community involvement for residents, families, staff, and external partners. This role oversees key initiatives including the Family Transition Program, Adult Day Program, and transitional services for admissions, ensuring alignment with the home’s mission, values, and person-centered care philosophy to enhance quality of life and inclusion.

 QUALIFICATIONS:

  • Minimum five (5) years of experience in gerontology, long-term care, or community program delivery.
  • Demonstrated experience in staff supervision, program coordination, and leadership.
  • Knowledge of applicable legislation, including the Fixing Long-Term Care Act, Resident Bill of Rights, and best practices in person-centered care.
  • Proven ability to build and maintain partnerships with internal and external stakeholders.
  • Strong communication, interpersonal, analytical, and conflict resolution skills.
  • A combination of education and experience may be considered.
  •  Valid Ontario driver’s licence.
