Social Service Worker - Family Transition Program

POSITION SUMMARY:
Provides support and counseling to Family Transition clients and acts as a resource to their families / caregivers, linking them with other community agencies where appropriate. The Social Service Worker acts as an advocate on behalf of clients to aid in improving their quality of life.
KEY ACCOUNTABILITIES
- Provides the following support:
- Provide mental and emotional support to assist clients and their families while awaiting Long-Term Care placement.
- Works collaboratively with a multidisciplinary team to communicate and track client progress.
- Liaise with Provincial Ministries, clients, family members, internal staff, community stakeholders and organizations, to provide educational sessions to support increasing awareness on issues affecting seniors.
- Liase with internal and external agencies and service providers to ensure the successful transition into a Long-Term Care Home.
- Evaluate specialized care plans through clinical assessments for clients and provide suggested interventions to meet the changing needs of clients.
- Ensures continuous quality improvement through the maintenance of records, reports and program statistics to support program evaluation.
- Liaise with clients, family members, the multidisciplinary team, internal and external stakeholders, and visitors to provide clients with access to applicable programs and services.
- Understanding of role specific health and safety responsibilities, and ensured compliance with internal and external safety policies, procedures, and legislation, including the Occupational Health and Safety Act.
- Performs other duties as assigned.
QUALIFICATIONS FOR SUCCESS
- Post-secondary education in social or human services or related discipline.
- Must be a Registered Social Service Worker in good standing with the Ontario College of Social Workers and Social Service Workers.
- Demonstrated knowledge of mental illness and treatment as it applies in a long-term care environment.
- Experience in counseling, assessment, and crisis intervention with an understanding of client difficulties related to economics, education, victims of family violence, physical or psychiatric impairments, geriatrics, addiction and isolation.
- Demonstrated ability to identify and collaborate with internal and external partners to deliver programs, services and outcomes.
- Knowledge of relevant trends and legislation, best practices and regulations including the Long-Term Care, Behavioural Supports Ontario and Occupational Health and Safety Act.
- Certification or additional training in mental health or responsive behaviours such as PIECES training, Crisis Prevention, Dementia Trainer and/or Consultant Certification, Teepa Snow – Positive approaches to care, Gentle Persuasive Approach or Mental Health Certificate is an asset.
- Ability to demonstrate the required competencies for this position in alignment with Oxford County’s competency framework.
- Valid Ontario driver’s license and a vehicle with insurance is required.
