POSITION SUMMARY:

Supports the effective and efficient delivery of Infection Prevention and Control (IPAC) practices at Woodingford Lodge in accordance with our vision, mission and value statements. 

KEY ACCOUNTABILITIES 

  • Coordinates and provides leadership to the IPAC program by:
    • Working with the interdisciplinary teams to implement the IPAC program.
    • Convening the Outbreak Management Team at the outset of an outbreak and regularly throughout an outbreak.
    • Convening the interdisciplinary IPAC Team at least quarterly and at a more frequent interval during an infection disease outbreak in the home.
    • Supporting the delivery of IPAC education to staff, caregivers, volunteers, visitors and residents.
    •  Participating as a member of the Continuous Quality Improvement (CQI)
      team.
    •  Implementing annual IPAC work plan and quality improvement action.
      items related to Ministry directives, standards and legislation.
    • Auditing IPAC practices within the home and implementing required.
      improvements identified through audits and trends.
    • Identifying challenges and opportunities for achieving best practices.
    •  Coordinating and maintaining a hand hygiene program for staff, residents, volunteers and caregivers.
    •  Ensuing all staff have access to up-to-date mask fit testing.
    •  Engage and consult with the Joint Health and Safety Committee on IPAC topics.
  • Liases with Provincial Ministries, residents, family members, internal staff, community stakeholders and organizations, and the public as necessary and appropriate. 
  • Responsible for supporting the home’s focus on Person and Family-Centred care by promoting and respecting residents in being an active partner of their health care. 
  • Understands role specific health and safety responsibilities, and ensures compliance with internal and external safety policies, procedures, and legislation, including the Occupational Health and Safety Act. 
  • Performs other duties as assigned.
  • RPN in good standing with the College of Nurses Ontario.

QUALIFICATIONS FOR SUCCESS:

  • Demonstrates experience implementing IPAC practices, continuous quality improvement training and education in infectious diseases, cleaning and disinfection, data collection and trends analysis, reporting protocols, outbreak management, asepsis, microbiology, adult education, epidemiology and program management.
  • An equivalent combination of education and experience may be considered.
  • Current CPR certificate is required.
  • Completion of or willingness to complete IPAC Core Competency training for Long-Term Care Homes.
  • Certification in or a willingness to achieve certification in Infection Control from the Certification Board of Infection Control and Epidemiology (CIC) within 2 years.
  • Completion of Joint Health and Safety Committee training, as approved by the Chief Prevention Office (CPO) of the Ministry of Labour is an asset.
  • Demonstrated capability to work independently with minimal supervision as well as collaboratively within a team to deliver programs, services and outcomes.
  • Proven ability to meet the required competencies for this position in alignment with Oxford County’s competency framework.
  • Demonstrated leadership and adult education abilities.
  • Valid Ontario driver’s license.

WORKING CONDITIONS:

  • Works in an office environment within a long-term care facility.
  • Driving between Woodingford Lodge satellite locations is expected.
  • Exposure to residents within the home, which may demonstrate responsive behaviours.
