POSITION SUMMARY:
Provide assessment and case management services to clients accessing services from the Human Services Department. This position delivers service around the following seven quality of life indicators: health, housing, income, employment, education & training, safety, and transportation. These indicators are integral in ensuring individuals are able to achieve full and equitable participation in their communities and it is the goal of the Department to assist individuals in improving their quality of life.
KEY ACCOUNTABILITIES:

Conducts interviews to assess clients needs/circumstances on behalf of Oxford County Human Services. Advises applicants of required documentation and/or their eligibility status within legislated and/or policy time lines.

Maintains a caseload; assesses ongoing eligibility for all cases, re-documenting cases at regular intervals as required by legislation and reassesses entitlement.

May conduct investigations and determine amounts of overpayments.
- Monitors and reports suspected cases of child abuse, parole/probation violations, by-law/Public Health infractions, etc.
- Conducts interviews to assess present living conditions. Obtains further information, often of a highly sensitive nature, regarding social history, involvement with social agencies or health care professionals.
- Creates and maintains confidential client files. May conducts random home visits to assess/evaluate client’s progress, identify personal, familial, social problems and takes appropriate action.
- Assists clients with accessing appropriate services/agencies. Processes referrals as necessary.
- Responds to extenuating emergencies and arranges for solutions on behalf of clients such as hydro shut downs, emergency assistance, fuel needs, etc.
- Calculates entitlement for department programs.
- Responds to complaints and inquiries from landlords, employers, etc., while respecting client’s rights to privacy and confidentiality.
- Liaises with appropriate Department staff, other County departments, and outside organizations.
- Meets goals/targets of Department as established by the County and/or provincial government ministries/departments.
- Assists with orientation of new staff.
- Performs other duties as assigned.
MINIMUM QUALIFICATIONS:

Diploma in a Social Services field.

Two years case management experience.

Knowledge of federal, provincial or municipal financial support programs.

Requires thorough knowledge of and ability to interpret policies, applicable legislation and regulations.

Excellent interpersonal and organizational skills and a professional work ethic.

Excellent computer skills and working knowledge of relevant software programs.

Knowledge of community resources.
- Valid Ontario driver’s license and a reliable vehicle with insurance coverage.
